Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services
 
Adult Day Services - Adult day program * social, recreational, and therapeutic activities for older adults and people with Alzheimer's disease and other related dementia in a group setting for part of the day * helps to support family caregivers who provide day-to-day care * meal provided * transportation for additional fee
 
Case Management - Offers information and support to people and their caregivers to assist them in remaining independent at home * short and long term planning
 
Community Dining - Social activities and a hot meal * group outings to restaurants and community sites
 
Friendly Visiting and Telephone Security Checks - Regular volunteer visits and/or phone calls to older adults and people with disabilities who live alone, to provide companionship and reassurance
 
Home and Property Support Services - Provides services such as lawn cutting, snow removal and handyman jobs (ie window cleaning, small home repairs, yard cleanup, etc)
 
Home Help and Personal Support - Assistance with daily chores such as laundry, light housekeeping, shopping together with the client and prescription pick up * assistance with personal care including bathing and transferring
 
Income Tax Clinic - Assists low-income older adults and adults with disabilities in filing their Income Tax forms during March and April * assistance provided by volunteers
 
Meals on Wheels - Hot meals delivered 5 days a week * special diet types available 
 
Respite Care - Offers relief to primary caregivers of frail and/or disabled adults who cannot be safely left alone
 
Supportive Housing - Support services available to tenants who live in the following buildings and meet the eligibility criteria:
 
Transportation - Rides to medical, shopping and social appointments for individuals who have mobility issues that prevent them from leaving their home safely
 
Weston/Mt-Dennis Site - Adult Day Services - Adult day program * social, recreational, and therapeutic activities for older adults and people with Alzheimer's disease and other related dementia in a group setting for part of the day * helps to support family caregivers who provide day-to-day care * meal provided * transportation for additional fee
