Home and Community Care - LHIN (Formerly CCAC) - Toronto Central

Health care services offered at home, at school, and in the community, as well as access to supported living programs and long-term care, are coordinated by Local Health Integration Networks (LHINs) to ensure people of all ages can make informed choices about their care, when and where they need it.

Home and community care services and staff have transferred from Community Care Access Centres (CCACs) to Local Health Integration Networks. Home and community care services are now provided through Ontario’s 14 Local Health Integration Networks.
