Service Description:
|
Provides comprehensive psychogeriatric assessments by a community clinician consulting with a geriatric psychiatrist (via OTN). The services offered by this program include a comprehensive psycho-geriatric assessment, treatment planning, education and follow-up. The IPOP team is interdisciplinary including an NP, RN, OT and Social Worker.
