Service Description:

Offers a range of programming that meets the level of support needs or unique challenges of specific populations. These include individual and shared accommodations, and staffed congregate settings for individuals needing daily support. Within that continuum, people with current involvement in the criminal justice system or those with several comorbid health issues are also served.

Referral is through Streamlined Access. Please call or the application form is available online.
After calling or applying online, an access worker will contact with the individual to establish eligibility and provide information about supports and services available.
