Service Description: Self-management, prevention, and education rehabilitation for people with a lung disease or condition * patients learn about smoking cessation, coping techniques, proper nutrition, and ways to better control of their fatigue and shortness of breath * patients attend weekly sessions for up to 3 times per week or a total of 36 sessions
