Assessment, treatment, rehabilitation, linkages to community organizations and support to individuals who have severe and persistent mental illness * interdisciplinary health care team consisting of addictions counsellors, occupational therapists, pharmacist, psychiatrists, registered nurses, peer support workers, vocational rehabilitation therapist, rehabilitation therapist, and social workers * clients may also be marginally housed, homeless, suffer from addictions, and HIV related disorders
Community Outreach Services - CATCH Homeless
- Connect homeless people presenting to hospital to comprehensive health supports including primary and psychiatric care, peer support and case management * collaboration between St Michael's Hospital, Inner City Health Associates and Toronto North Support Services
