Service Description:

Help older adults leaving hospitals to arrive home * offer clients additional support for up to 12 weeks after transitioning home from hospital
Services include:- assistance to book appointments and associated transportation
- appointment reminders
- medication reminders
- safety checks

Must have member of professional care team in hospital to complete a Home At Last referral form. For referral form, go on website

Eligibility / Target Population:
|
Adults aged 55 years or older leaving hospital emergency department or inpatients units, without support from friends or family upon discharge, and able to direct their own care
